Job summary

Bethesda Health Group in Belleville, IL is seeking an experienced LPN to provide essential nursing care, conduct resident assessments, and communicate changes to the IDT. You will document findings, participate in care planning, and ensure safe medication administration and timely interventions.

Qualifications

• Education from an accredited school of nursing that results in becoming a Licensed Practical Nurse.
• Current license as an LPN in the state of Illinois.
• IV Certification required or willing to obtain within six months.
• CPR/AED Certification required or willing to obtain within six months.
• Ability to assess basic nursing care needs of geriatric residents.
• Strong communication with residents, families and staff.

Responsibilities

• Provides basic nursing care and assists in resident assessments on admission and ongoing.
• Communicates significant changes to the interdisciplinary team and documents assessments.
• Active participation in the care planning process and updates to the care plan as interventions change.
• Implements nursing interventions, administers medications and treatments safely and timely.
• Administers IV fluids only if IV certified and notifies physician using SBARR when needed.
• Maintains composure during crises and uses QAPI to address issues.
