Job summary

Bethesda Health Group in Illinois seeks a Licensed Practical Nurse to provide basic nursing care, assess residents on admission and ongoing, and participate in care planning.

You will document assessments, communicate changes to the IDT and families, administer medications, monitor the care plan, and support resident and staff education. The role requires strong critical thinking and excellent communication skills.

Qualifications

• Education from an accredited school of nursing resulting in LPN licensure.
• Long-term care / Rehab / Assisted Living experience preferred.
• Current license in the state of employment as LPN.
• IV Certification required or willing to obtain within six months.
• Current CPR/AED Certification required or willing to obtain within six months.
• Ability to assess basic nursing needs of geriatric residents.
• Excellent communication and organizational skills.
• Strong critical thinking skills.

Responsibilities

• Provide basic nursing care and assess residents on admission and ongoing.
• Document assessments and communicate changes to the IDT and families.
• Implement nursing interventions per care plan including medications and treatments.
• Monitor care plan delivery and address problems via QAPI process.
• Assist with ADLs and participate in continuing education.
