Job summary

Bethesda Health Group in Illinois is seeking an LPN to provide basic nursing care, assess residents on admission and ongoing basis, and communicate changes to the IDT. You will participate in care planning and document assessments, updating plans as needed while ensuring safe medication administration and treatments.

The role requires current LPN licensure, CPR/AED and IV certification (or willingness to obtain within six months), and strong communication and time-management skills.

Qualifications

• Education from an accredited school of nursing resulting in LPN license.
• Current LPN license in Illinois.
• CPR and AED certification required or be willing to obtain within six months.
• IV certification required or willing to obtain within six months.
• Long-term care experience preferred.

Responsibilities

• Provide basic nursing care and assess residents on admission and ongoing basis.
• Document assessments accurately and communicate changes to the IDT.
• Participate in care planning and update plans as interventions require.
• Ensure timely and safe medication administration and treatments.
• Utilize SBARR for physician communication during crises and updates.
• Document interventions electronically and in writing.
