Job summary

Bethesda Health is seeking a Licensed Practical Nurse to provide basic nursing care, assess residents on admission and throughout their stay, and communicate changes to the interdisciplinary team. The role includes updating care plans, administering medications, and timely documentation in both electronic and written form.

Responsibilities

• Provides basic nursing care by assisting in the assessment of the resident on admission and ongoing basis.
• Communicates significant changes in condition to the IDT and documents assessments.
• Participates in care planning and updates the plan of care as interventions revise.
• Delivers nursing interventions including medication administration; IV care if certified.
• Documents interventions accurately and timely; uses SBARR for physician communication.
