Job summary

Midwest Health, Inc. is seeking a Licensed Practical Nurse to provide direct nursing care to residents in our facility. The role requires an active LPN license in good standing, CPR certification, and competent English communication.

You will follow federal, state, and local guidelines and work under the direction of the Nurse Supervisor. Responsibilities include assisting in admissions/discharges, daily hygiene, charting, administering routine procedures, and maintaining resident

Qualifications

• Must have a valid professional license (LPN) in good standing with the State.
• Must have a valid CPR certification.
• Must have basic computer skills; read, write, speak, and understand English.
• Must be able to work beyond normal hours, including weekends/holidays.

Responsibilities

• Provide direct nursing care to residents per current standards and regulations.
• Comply with facility policies and nursing department procedures.
• Report abuse/neglect to Administrator or Nursing Supervisor immediately.
• Maintain confidentiality of resident information; document changes in condition.
• Assist in admission, discharge, and daily hygiene care (bathing, grooming, toileting).
• Assist with mobility and transfers; use transfer belts as needed.
• Administer routine procedures (catheterization, tube feedings, dressings, etc.) as required.
• Perform routine charting, vitals, and other resident care tasks per plan.
• Develop good rapport with staff and inter-departmental teams.
• Complete required documentation and reports for resident care and incidents.
