Job summary

Midwest Health, Inc. is seeking a Licensed Practical Nurse to provide direct nursing care to residents in our community, ensuring compliance with federal, state, and local standards as directed by RCC Services or the Nurse Supervisor to maintain the highest level of quality care.

The nurse will support admissions and discharges, assist with daily hygiene, monitor residents, document changes, and collaborate with the nursing team to deliver comprehensive care.

Qualifications

• Provide direct nursing care to residents in accordance with federal, state, and local standards.
• Maintain confidentiality of resident and family information.
• Administer professional nursing services as required.

Responsibilities

• Comply with Community policies, procedures and safety rules.
• Comply with nursing department policies and procedures.
• Report abuse/neglect to the Executive Director or Unit Manager.
• Notify Nurse Supervisor of changes in a resident’s condition or behavior.
• Assist in admission and discharge of residents.
• Assist residents with daily hygiene care including bathing and personal care.
• Assist with bowel/bladder functions and specimen collection.
• Transport residents to appointments and activities.
• Assist with transferring and positioning residents.
• Document changes and care provided in the resident’s chart.
