Midwest Health, Inc. is seeking an Licensed Practical Nurse to provide direct nursing care to residents in a facility setting.

You will ensure care aligns with federal, state, and local standards and is overseen by the Director of Nursing or Unit Manager. Responsibilities include administering clinical services, documenting care, and reporting changes promptly, while upholding residents' dignity and confidentiality.

Formación

• Must have valid LPN license in good standing with the state.
• Must have valid CPR certification.
• Must have basic computer skills; read, write, speak, and understand English.

Responsabilidades

• Provide direct nursing care to residents in accordance with current standards and facility policies.
• Report witnessed or reported abuse/neglect immediately to the Administrator, Director of Nursing or Unit Manager.
• Maintain confidentiality of resident and family information and document changes in resident condition.
• Assist in admission, discharge, and daily hygiene care, including bathing, grooming, and mobility assistance.
• Transport residents to appointments and activities; assist with transferring and positioning.
• Perform routine charting and maintain accurate medical records and incident reports.
• Assist with care of dying residents and post-mortem care as required.
• Adhere to safety procedures and report occupational exposures per policy.
