Job summary

You will support admissions, discharges, daily living activities, and ensure accurate documentation of resident status. The role requires strong communication, compassion, and the ability to work with diverse teams in a structured facility

Qualifications

• Must hold a valid LPN license in good standing with the state.
• Experience delivering direct nursing care to residents.
• Strong documentation and communication skills.

Responsibilities

• Provide direct nursing care to residents while adhering to federal, state, and local standards.
• Follow nursing department policies and procedures and report concerns to supervisors.
• Assist with admission, discharge, hygiene, and daily living activities for residents.
• Document changes in resident condition and communicate with the care team.
• Assist with transfers, transport, and routine procedures per the care plan.
• Complete charting and administrative duties related to resident care.
