Ennoble Care is seeking a full-time Patient Care Coordinator to support clinicians, staff and patients from our Marlton, New Jersey office. The role focuses on coordinating care, scheduling, and ensuring high-quality service while maintaining open communication with care teams.

The ideal candidate is compassionate, highly organized, and able to multitask in a fast-paced environment. Expect on-site duties, with 8:30am–5:00pm hours, and a starting pay of $18.00 per hour.

Formación

• Must be comfortable with speaking on the phone for large amounts of the day.
• Compassionate and empathetic towards patients with exceptional customer service.
• Able to take accurate notes and document tasks promptly.

Responsabilidades

• Create individualized patient care plans and perform care coordination using the EMR system.
• Maintain frequent contact with patients to provide care coordination and ensure program compliance.
• Document all client communications accurately and relay information to clinicians as needed.
• Communicate with Primary Care Clinician or designee regarding patient needs and care plans.
