Ennoble Care is seeking a full-time Care Coordinator, Inbound (Call Center Representative) to join our Marlton, New Jersey team. You will support patients and clinicians, maintain clear communication, and help manage care programs to achieve positive outcomes.

Responsibilities include documenting interactions, coordinating with primary care providers on emergent needs, and using EMR systems to ensure accurate, comprehensive care plans. A patient‑focused, empathetic approach is essential.

Qualifications

• Must be comfortable speaking on the phone for long periods.
• Must be compassionate and empathetic toward patients with exceptional service.
• Ability to take accurate notes documenting each task promptly.
• Ability to multitask between patients and workstreams while staying organized.
• Thrive in a fast-paced environment.
• Proficient with computer systems including Outlook and MS Office.

Responsibilities

• Maintain frequent contact with patients to provide support and monitor care programs.
• Document all client communications accurately.
• Coordinate with Primary Care Clinician on emergent needs and care plans.
• Collaborate across departments to meet patient needs.
• Use technology including secure email, phone system, EMR.
• Follow documentation protocols and daily work logs.
• Deliver excellent customer service and support the team.
