Job summary

PACE Southeast Michigan seeks a compassionate Nursing Assistant to provide person-centered care in Sterling Heights, MI. The PCA supports participants in home, community, or day health settings, assists with activities of daily living, and helps maintain safety and comfort under CMS guidelines.

Qualifications

• HS diploma or GED required.
• Valid Michigan Driver's license.
• Current CPR card required.
• Certification is optional though preferred, but must have one year experience with a frail or elderly population.
• Experience in long-term care, hospital, or home care setting.
• Must have received certificate of completion from a formal training program outside of PACE SEMI.

Responsibilities

• Provide supportive services to participants in home, community, or day health settings as outlined in the Care Plan.
• Assist with activities of daily living, meal preparation, toileting, and mobility as needed.
• Document care accurately on visits and adjust care plans with RNCM guidance.
• Report deviations from schedule and participate in mandatory in-service training.
• Maintain safety, infection control, and regulatory compliance during all care activities.
