Job summary

PACE Southeast Michigan seeks a Direct Home Care Worker to provide essential in-home support to participants in the Westland area. You will help with daily activities, meals, mobility, and transportation, while maintaining careful documentation per the plan of care.

The role requires a High School diploma or GED and formal Direct Care/Nurse Assistant training or equivalent experience. Strong communication and empathy are essential to support independence for participants with varying needs.

Qualifications

• Minimum High School Diploma or GED required.
• Completion of Direct Care/Nurse Assistant training preferred.
• Ability to document visits accurately and timely.

Responsibilities

• Provide in-home and community support to participants.
• Assist with activities of daily living and meal preparation.
• Remind and assist with medications as directed.
• Transport and accompany to appointments as needed.
