Job summary

PACE Southeast Michgan is seeking a Participant Care Associate to provide in-home and community-based support to frail elders, including transportation and appointment assistance. You will follow the PCA/CNA Care Plan, document visits, and coordinate with nurses to ensure quality care.

The role requires a high school diploma or GED, a valid Michigan driver’s license, CPR certification, and at least one year of experience with the elderly in long-term care, hospital, or home care settings.

Qualifications

• H.S. diploma or GED required.
• Valid Michigan Driver's license.
• Current CPR card required.
• Certification is optional though preferred, but must have one (1) year experience with a frail or elderly population.
• Experience must be in a long-term care, hospital, or home care setting.
• Must have received a certificate of completion from a formal training program outside of PACE SEMI.

Responsibilities

• Provide in-home or community-based care to frail elders as outlined in the PCA/CNA Care Plan.
• Assist with activities of daily living and transportation as needed.
• Document care provided on each visit and communicate progress to RNCM or clinic nurse.
• Respond to emergencies according to policies and procedures.
• Maintain scheduling flexibility and respectful interactions with diverse participants.
