Job summary

PACE Southeast Michigan is seeking a Direct Home Care Worker to provide in-home and community-based supportive services to participants. Responsibilities include personal care, meal preparation, medication reminders, transportation, and accurate visit documentation.

Emphasis on maintaining participant independence, working with dementia patients, and adhering to CMS guidelines. Required are a high school diploma or GED, at least 2 years of relevant experience, and completion of a direct care

Qualifications

• High School Diploma or GED required.
• Minimum 2 years of relevant work experience.
• Formal training program in Direct Care, Nurse Assistant, or Medical Assistant required.
• In-service training annually (infection control, TB testing, safety, BLS).
• Must understand and maintain infection control and CMS competencies.
• Must be medically cleared for communicable diseases and up-to-date immunizations.
• Ability to work with diverse populations and individuals with dementia.

Responsibilities

• Provide personal care and assistance with activities of daily living.
• Assist with meal preparation and feeding; toileting and incontinence support.
• Assist with shopping, homemaking, laundry, and errands.
• Provide transportation and accompany to appointments; document visits timely.
• Maintain accurate documentation for each visit; communicate with RNCM/clinic nurse.
• Respond to emergencies per policy; report deviations from schedule to supervisor.
