Job summary

PACE Southeast Michigan is seeking a Direct Home Care Worker to provide compassionate, in-home care and community support. You will assist with daily living activities, transportation, and day health center visits, following each participant's Care Plan.

You will document services, coordinate with RNCM, and adhere to CMS guidelines while maintaining flexibility to meet participants' needs in various environments.

Qualifications

• Minimum High School Diploma or GED required.
• Must have minimum of 2 years of relevant work experience.
• Must have completed formal Direct Care/Nurse Assistant training outside of PACE SEMI.
• Annual in-service training including infection control, TB testing, safety, and BLS.
• Ability to establish and maintain relationships with participants and co-workers.
• Organize and maintain flexible scheduling for job responsibilities.
• Ability to work with diverse ethnic and cultural backgrounds.
• Ability to relate to participants with dementia or developmental disabilities.
• Must meet CMS competencies before working independently.
• Must be medically cleared for communicable diseases and up-to-date immunizations.

Responsibilities

• Personal care and assistance with activities of daily living.

• Medication reminders and light housekeeping.

• Documentation of services provided during each visit.
