Heartland Hospice is seeking a dedicated Registered Nurse Case Manager to provide in-home hospice and palliative care. You will lead comprehensive assessments, develop personalized plans of care, and coordinate with an interdisciplinary team to ensure compassionate, patient-centered support.

You will educate patients and families, monitor symptoms, and document care with attention to regulatory requirements. This role emphasizes empathy, communication, and timely care delivery.

Qualifikationen

• RN license for state of employment is required.
• CPR certification is required.
• Current automobile insurance and valid driver’s license.

Aufgaben

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients, families, and caregivers about disease, end-of-life care and medications.
• Document timely medical records and progress notes.
