Job summary

Care Initiatives Hospice is seeking a compassionate Registered Nurse (RN) Case Manager to deliver comprehensive, patient-centered care at home or in settings like long-term care, assisted living, or hospital.

As an RN Case Manager, you will assess patients, develop and monitor plans of care, coordinate with the physician and interdisciplinary team, and support families with education and counseling.

Qualifications

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Two years of verifiable nursing experience (RN/LPN) preferred.
• Current driver's license and ability to travel within the region.
• Home internet access.
• Ability to obtain Hospice and Palliative RN Certification (CHPN) within 24 months.

Responsibilities

• Use clinical nursing knowledge to deliver high quality patient care in the patient’s residence.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive assessments on all patients as per policies.
• Provide continuous visits and interdisciplinary assessments of the patient.
• Deliver care with clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care to meet patient-specific needs.
