Job summary

Heartland Hospice is seeking a registered nurse to join our team as an RN Case Manager, providing in-home hospice care and coordinating with the IDG to deliver compassionate, patient-centered services.

You will perform comprehensive assessments, develop plans of care, educate patients and families, and ensure timely documentation and compliance while supporting quality improvement initiatives.

Qualifications

• Registered Nurse with hospice/palliative care experience preferred.
• Experience performing comprehensive patient assessments and developing care plans.
• Strong documentation and compliance with regulatory standards.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver nursing interventions and symptom management.
• Develop and update individualized plans of care and visit frequencies.
• Coordinate care with interdisciplinary teams and physicians.
• Educate patients, families, and caregivers on disease, self-care, and end-of-life care.
• Communicate changes in patient condition to the care team.
• Document timely clinical records and support on-call activities.
