Job summary

Care Initiatives Hospice is seeking a compassionate RN Case Manager to provide comprehensive, patient- and family-centered care in patients' homes or other settings. You will assess, plan, and coordinate care with the physician and interdisciplinary team to meet physical, emotional, and spiritual needs.

As an RN Case Manager, you will conduct initial and ongoing assessments, develop and update the plan of care, document clearly, and support patients and families through the hospice journey,

Qualifications

• Active Iowa RN license is required.
• Hospice or palliative care experience preferred.
• CPR certification required.
• Two years nursing experience preferred.
• Valid driver’s license and ability to travel within the region.

Responsibilities

• Use nursing knowledge and assessments to deliver high quality patient care at the patient’s residence.
• Develop and oversee the plan of care under physician direction with interdisciplinary involvement.
• Perform initial and ongoing patient assessments per policies.
• Provide continuous visits and interdisciplinary review of the patient’s needs.
• Deliver care with clear, concise clinical documentation.
• Counsel and educate patients, caregivers and families as indicated.
• Update the plan of care to meet patient-specific needs.
