Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing.
• 1–2 years of public health/home care/hospice experience preferred.
• Management experience not required.
• Background checks and references required.
• Excellent observation and communication skills; basic math; nursing skills per competency checklist.
• Prolonged walking/standing; ability to lift and transfer patients.
• Licensed driver with insured vehicle.
• CPR certification required.

Responsibilities

• Complete initial and ongoing assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Write and revise the plan of care with patient and family.
• Administer medications and treatments as prescribed.
• Counsel patient and family; provide health instructions.
• Act as Case Manager for assigned caseload and coordinate care.
• Document accurate clinical notes; report changes to physician.
• Coordinate care with the interdisciplinary team; attend meetings.
• Participate in on-call duties; arrange equipment and services.
• Supervise ancillary personnel as required.
