Job summary

Affinity Health Management in Alliance, OH is seeking a dedicated hospice aide to support patients under RN supervision. The role focuses on personal care, safety, comfort, and accurate observation reporting according to each patient's care plan.

You will assist with bathing, grooming, feeding, mobility, and basic housekeeping while maintaining compassion, confidentiality, and compliance with state regulations. A CNA certificate and CPR are preferred; covid vaccination required per policy.

Qualifications

• Completion of state-approved CNA course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on verification).
• COVID-19 vaccination as per facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and comfort with terminally ill care.
• 1 year home health/hospice experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years old.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care per patient assignment and respond promptly.
• Communicate effectively with patients and families while maintaining confidentiality.
• Ensure patient safety and use equipment properly.
• Provide baths, back rubs, oral hygiene, shampoo, and change linens.
• Assist with dressing and undressing patients.
• Assist feeding as assigned; no homemade foods.
• Monitor skin condition to prevent tissue breakdown.
• Report pain or changes to RN Case Manager.
• Assist in ambulation and exercise as directed.
• Provide range of motion and positioning assistance.
• Assist with self-administered meds as allowed by state rules.
• Offer bedpans/urinals and light housekeeping tasks.
• Perform ROM and simple procedures with competency.
• Provide respite for patient/family when on-site.
• Keep living area clean/ordered.
• Follow documentation and care procedures and codes.
• Participate in quality improvement teams.
• Be respectful of patient/family environment.
• As mandatory reporter, report non-compliance to health dept per statutes within 14 days.
• Report suspected abuse/neglect to appropriate authorities immediately.
