Job summary

Affinity Health Management in Warren, Ohio is seeking a hospice aide to join our team under the supervision of a registered nurse. You will perform tasks outlined in the patient’s plan of care to promote comfort and report observations to ensure high-quality, continuous, and safe patient care.

Responsibilities include personal care, aiding in feeding and mobility, monitoring skin health, and documenting care in the EMR.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Covid vaccination required due to facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and compassionate end-of-life care.
• Prior hospice/long-term care experience preferred (1 year).
• Basic computer skills to document in EMR.

Responsibilities

• Communicate positively with patient and family, maintaining confidentiality.
• Ensure safety and proper use of equipment.
• Assist with bathing, hygiene, and changing bed linens.
• Assist in dressing, feeding, and mobility as directed.
