Job summary

Reserve Care (formerly Hospice of the Western Reserve) seeks a part-time Registered Nurse (RN) for day shift in an inpatient facility in Sandusky, OH. You will deliver prescribed medical treatments, assess patient needs, and coordinate care with the interdisciplinary team.

Responsibilities include monitoring symptoms, providing emotional support to patients and families, and participating in rounds, case reviews, and discharge planning as required. Active OH RN license and CPR are essential.

Qualifications

• Active Ohio RN license.
• CPR certification required.
• Hospice or palliative care experience preferred.

Responsibilities

• Provide intermittent skilled nursing to terminally ill patients under physician direction.
• Assess physical, psychosocial, and spiritual needs of patient and family/caregivers.
• Coordinate Plan of Care with the transdisciplinary team.
• Monitor pain and symptoms; inform patient/family on management.
• Communicate with physicians and case managers as needed.
• Participate in discharge planning and care conferences.
