Job summary

Reserve Care seeks a Registered Nurse (RN) for a part-time day shift at our inpatient facility in Sandusky, Ohio. You will work with a transdisciplinary team to provide prescribed medical treatment and personal care to terminally ill patients, coordinating each patient’s Plan of Care using the Nursing Process.

Responsibilities include assessing needs, managing pain and symptoms, guiding families, and participating in on-call and discharge planning.

Qualifications

• Licensed RN in State of Ohio.
• Current CPR certification.
• Experience in hospice care, home health, oncology, or pain/symptom management preferred.
• Must have and maintain a valid driver’s license.
• Hospice and Palliative Care Certification desired; to be completed within first 2 years of hire.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients with a transdisciplinary team.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Provide intermittent skilled nursing services under physician direction.
• Monitor pain and symptoms; inform and assist patient and family in management.
• Participate in care conferences, discharge planning, and on-call rotation.
