Job summary

Reserve Care is seeking a Registered Nurse to join the 3rd shift on our Inpatient Unit in Sandusky, OH. You will work within a transdisciplinary team to implement and monitor the Plan of Care for terminally ill patients and coordinate care with physicians.

The role requires an Ohio-licensed RN, CPR certification, and a commitment to compassionate, high-quality care in a hospice setting. On-call and discharge planning duties are part of the responsibilities.

Qualifications

• Must be licensed as a Registered Nurse in Ohio.
• CPR certification is required.
• Experience in hospice, palliative care, or home health is preferred.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients as part of the care team.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Administer intermittent skilled nursing services under physician direction.
• Monitor pain and symptoms; support patients and families in management.
