Job summary

Reserve Care in Sandusky, OH is seeking a licensed RN to provide prescribed medical treatment and personal care to terminally ill patients. You will collaborate with the transdisciplinary team to implement the Plan of Care and coordinate care across settings.

Responsibilities include assessing needs, monitoring pain and symptoms, educating patients and families, participating in care conferences, and ensuring compliance with clinical standards.

Qualifications

• Licensed RN in State of Ohio with current CPR certification.
• Experience in hospice, home health or palliative care preferred.
• Must have and maintain a valid driver’s license and reliable vehicle.

Responsibilities

• Provide prescribed medical treatment and personal care services to terminally ill patients within the transdisciplinary team.
• Coordinate Plan of Care implementation, monitor pain and symptoms, and educate patients and families.
• Participate in care conferences, bereavement follow-up and discharge planning as needed.
• Maintain clinical documentation and communicate with physicians and case managers.
