Job summary

Reserve Care, formerly Hospice of the Western Reserve, seeks an experienced Registered Nurse to join our hospice team in Sandusky, OH. You will deliver prescribed care in collaboration with the interdisciplinary team and coordinate plans of care for each patient.

Responsibilities include assessments, symptom management, family education, and participating in care conferences. Ohio nursing license, CPR, and a valid driver’s license are required; hospice certification is preferred and must be

Qualifications

• Licensed RN in Ohio.
• Experience in hospice care, home health, oncology, or pain and symptom management preferred.
• Current CPR certification.
• Valid driver’s license with good driving record and auto insurance.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Provide intermittent skilled nursing services under physician direction.
• Monitor pain and symptoms and assist patients and families in management.
