Three Oaks Hospice in Rolling Meadows, IL is seeking a compassionate Registered Nurse to administer skilled nursing care for patients in their homes. Coordinate care with the interdisciplinary team, patients, families, and referring agencies, and manage care plans.

The role requires an Associate or Bachelor’s degree in Nursing, current IL RN license, CPR certification, and at least two years of clinical RN experience (hospice/home health preferred).

Formación

• RN degree and licensure required
• Two years’ RN experience; hospice/home health preferred
• CPR certification required
• Proficient with computer/Office software

Responsabilidades

• Initial and ongoing nursing assessments.
• Coordinate care with IDT and family.
• Educate patients/families on disease process and care.
• Initiate appropriate nursing procedures.
• Document progress and plan updates.
• Coordinate services and prioritize needs with IDT.
• Inform physician of changes and outcomes.
• Determine scope and frequency of services based on acuity.
• Assess caregiver ability to meet needs.
• Participate in on-call coverage.
