Job summary

Three Oaks Hospice, Inc. is seeking a Registered Nurse to administer skilled nursing care for clients in their homes.

You will coordinate with the interdisciplinary team, patient and family, and referring agencies to plan and deliver compassionate end-of-life care. The role requires a nursing degree, an active RN license, CPR certification, and at least two years of clinical RN experience (hospice/home health preferred).

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and a valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patients and families.
• Demonstrate nursing practice skills and ability to cope with family emotional stress.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Educate patient/family on disease process, self-care, and end-of-life care; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical notes and progress reports reflecting goals achieved.
• Coordinate all patient/family services and manage referrals as needed.
• Communicate changes in patient needs to physicians and team members.
• Determine scope and frequency of services based on acuity and needs.
• Provide on-call coverage for patients and families as assigned.
