Three Oaks Hospice, Inc. in Illinois is hiring a Registered Nurse to provide expert hospice care to patients in their homes and settings. You will coordinate with the interdisciplinary team, assess needs, and implement care plans to improve quality of life at end of life.

The role requires a current state RN license, CPR certification, and strong communication and organizational skills. Experience in hospice or home health is preferred, with on-call duties as part of the schedule.

Qualifications

• Associate or bachelor’s degree in Nursing from an accredited program.
• Current RN license in the state and a valid driver's license.
• Two years’ RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; MS Office experience preferred.
• Strong interpersonal, communication, and organizational skills.
• High standard of ethics and compliance.

Responsabilités

• Perform initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs.
• Implement the individualized plan of care and revise as necessary.
• Educate patients and families about disease, self-care, end-of-life care; provide training to staff.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes showing progress toward goals.
• Coordinate patient/family services with the interdisciplinary team.
• Use case management approach and refer to other services as needed.
• Inform physician and staff of changes in patient needs and outcomes.
• Determine scope and frequency of services based on acuity.
• Assess caregiver ability to meet patient needs and provide support.
• Communicate information using current organizational processes and technology.
• Apply admission and recertification criteria for hospice care.
• Perform on-call responsibilities and provide on-call service.
• Other duties as assigned.
