Job summary

Three Oaks Hospice is seeking a registered nurse to administer skilled nursing care to patients in their homes, coordinate care with the IDT and families, and manage end-of-life care planning.

Qualifications

• Associate or bachelor’s degree in Nursing from an accredited program.
• Current RN license in the state and valid driver’s license.
• Minimum 2 years of RN experience in clinical care; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.
• Ethics and professional conduct in care provision.

Responsibilities

• Conduct initial and ongoing comprehensive nursing assessments in patients' homes.
• Coordinate care with the interdisciplinary team and families.
• Provide end-of-life care planning, education, and support.
• Prepare progress notes and document patient status.
• Manage on-call responsibilities as assigned.
• Utilize case management approaches to coordinate services.
