Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice care in home and community settings. You will assess patients, develop care plans, administer prescribed medications, and educate families to support comfort and quality of life.

This role emphasizes collaboration with the interdisciplinary team, on-call coverage, and ongoing professional development to meet the evolving needs of patients and families in home-based hospice care.

Qualifications

• Graduate of a National League for Nursing accredited school of nursing.
• Current state RN licensure in good standing.
• Minimum 1 year as a professional nurse within the last 3 years or a BSN with RN license; 2 years in public health, home care, or hospice preferred.
• Supervising hospice aides not required.

Responsibilities

• Performs comprehensive patient and family assessments to determine hospice needs.
• Develops and updates care plans and coordinates care with the interdisciplinary team.
• Administers medications and treatments as prescribed by the physician.
• Educates patients and families on self-care, comfort, and disease management.
• Participates in on-call duties and ensures continuity of care.
