Job summary

Swan Hospice is actively seeking a Registered Nurse (RN) to join our team in New Jersey. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes.

As an RN, you will oversee the primary caseload, perform assessments, implement and adjust the care plan with the IDT, and educate families and staff on nursing care and symptom management in a home-care/hospice setting.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor's degree in Nursing preferred.
• CHPN certification preferred.

Responsibilities

• Oversees primary caseload of hospice patients and provides skilled professional nursing care as prescribed by the physician.
• Performs initial and comprehensive nursing assessments and updates the plan of care with the IDT.
• Educates, supervises, and counsels patients, family members, and staff on nursing care and home care needs.
• Collaborates with the interdisciplinary team to ensure patient and family needs are met; supervises Hospice Aides/Homemakers.
