Job summary

Swan Hospice is seeking an experienced Registered Nurse to manage a primary caseload of hospice patients in New Jersey, including home care and facility settings. You will perform nursing assessments, develop and implement the care plan with the Interdisciplinary Team, and educate patients and families about pain management and comfort measures.

Applicants must be a graduate of an accredited RN program, NJ RN license, and have at least 1 year of general nursing experience with hospice or home

Qualifications

• Graduate of an accredited registered nursing program.
• 1 year general nursing experience preferred (hospice/home health).
• Experience with pain and symptom management preferred.
• Ability to work within an interdisciplinary team.
• CHPN preferred

Responsibilities

• Oversees primary caseload of hospice patients and provides nursing care.
• Provides initial and comprehensive nursing assessments and plans of care with IDT.
• Educates patients, families, and caregivers on nursing care and comfort measures.
• Teaches and supervises Hospice Aides/Homemakers.
• Collaborates with IDT to ensure patient needs are met.
