Job summary

Swan Hospice in Central New Jersey is seeking Registered Nurses (RN) to join our team as Full/Part Time on-call and runners. We provide hospice care to patients in skilled nursing facilities, assisted living facilities, and private homes.

The RN will oversee a primary caseload, perform comprehensive nursing assessments, develop and implement the plan of care with the IDT, and educate patients and families. Leadership and on-call duties are included.

Qualifications

• Graduate of an accredited registered nursing program.
• Current NJ RN license.
• Bachelor’s degree in Nursing preferred.
• CHPN certification preferred.

Responsibilities

• Oversees primary hospice patient caseload and provides skilled nursing care as prescribed by the physician.
• Performs initial and comprehensive nursing assessments and develops IDT care plans with the team.
• Educates and counsels patients, families, and caregivers on nursing care and home care needs.
• Leads and mentors Hospice Aides/Homemakers and collaborates with IDT to meet patient goals.
• Manages documentation, coordination, and communication to ensure continuity of care.
• Participates in on-call coverage and supports leadership roles as needed.

Education

Graduate of an accredited RN program

Bachelor’s degree in Nursing (preferred)
