Job summary

Swan Hospice is actively seeking Registered Nurses (RN) to join our team in New Jersey. We provide comprehensive hospice care across skilled nursing facilities, assisted living facilities, and private homes.

The RN will oversee a primary caseload, conduct assessments, implement and adjust the plan of care, and educate families and staff. NJ RN licensure and a valid driver's license are required; CHPN certification is preferred.

Qualifications

• Graduate of an accredited registered nursing program.
• Minimum of one (1) year general nursing experience; hospice/home health/medical/surgical/ICU preferred.
• Current New Jersey RN licensure and satisfactory references.
• Valid driver’s license and reliable transportation as required by NJ regulations.

Responsibilities

• Oversee primary caseload of Hospice patients and provide skilled nursing care.
• Perform initial and ongoing nursing assessments and develop the plan of care with the IDT.
• Implement and reevaluate the plan of care to meet patient needs and goals.
• Educate and supervise patient, family, and staff regarding care and nursing needs.
• Collaborate with the interdisciplinary team to ensure comprehensive, compassionate hospice care.
• Supervise Hospice Aides/Homemakers as part of the care team.
