Job summary

Swan Hospice in New Jersey seeks Registered Nurses (RN) to provide skilled nursing care to patients in skilled nursing facilities, assisted living facilities and private homes. You will conduct initial and ongoing nursing assessments, develop and update the care plan with the interdisciplinary team, and educate families while supervising aides to ensure high-quality hospice care.

Active NJ RN licensure and driving eligibility are required; BSN and CHPN are preferred.

Qualifications

• Graduate of an accredited registered nursing program.
• BSN preferred.
• CHPN certification preferred.

Responsibilities

• Oversees primary caseload of hospice patients and provides skilled nursing care.
• Performs initial and comprehensive nursing assessments and schedules IDT plans.
• Reassesses and revises the care plan based on patient needs.
• Educates and counsels patient families and supervises Hospice Aides/Homemakers.
• Collaborates with IDT to ensure all care needs are met.
