Job summary

Swan Hospice is seeking caring Registered Nurses (RN) to join our team in Central New Jersey on a full-time, part-time on-call and runner basis. You will provide skilled nursing care to hospice patients in homes and facilities, following physician orders and hospice guidelines.

As a core member of the Interdisciplinary Team, you will assess patients, create and implement care plans, educate families, supervise aides, and maintain accurate documentation to promote comfort and quality of life.

Qualifications

• Licensed RN in good standing with state board.
• Experience in hospice or palliative care preferred.
• Ability to work autonomously and within an IDT.

Responsibilities

• Oversees primary caseload of Hospice patients and provides skilled professional nursing care.
• Conduct initial and comprehensive nursing assessments for each patient.
• Collaborates with IDT to develop, implement and revise the plan of care.
• Supervises Hospice Aides/Homemakers and provides coaching as needed.
• Documents accurate clinical notes and maintains timely records.
