Job summary

Swan Hospice in New Jersey is seeking Registered Nurses to join our team, delivering quality hospice care in skilled nursing facilities, assisted living facilities, and private homes.

The RN will manage a primary caseload, perform initial and ongoing nursing assessments, collaborate with the Interdisciplinary Team to plan and revise care, educate patients and families, and supervise aides. Valid NJ RN licensure and commitment to compassionate end‑of‑life care are required.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• CHPN certification preferred.
• Minimum 1 year general nursing experience; hospice/home health/medical/surgical/critical care preferred.
• Experience with pain and symptom management preferred.
• Ability to work within an interdisciplinary setting.
• Understanding of hospice philosophy and end-of-life care.

Responsibilities

• Oversees primary caseload of hospice patients and provides skilled nursing care as prescribed.
• Performs initial and comprehensive nursing assessments and assists in developing the IDT plan of care.
• Implements and reevaluates the plan of care to meet patient needs.
• Teaches, supervises, and counsels patients, families, and staff; collaborates with IDT to ensure patient and family care needs are met.
