Job summary

Swan Hospice in New Jersey seeks Registered Nurses (RN) to join our hospice team. You will oversee primary caseloads, provide skilled nursing care, and collaborate with the interdisciplinary team to meet patient and family needs.

Requirements include an accredited RN program and NJ licensure; CHPN is preferred. Roles cover home, facility, and facility-based care with supervision of aides/homemakers.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor's degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.

Responsibilities

• Oversees primary caseload of Hospice patients and provides skilled nursing care.
• Implements and reevaluates the plan of care with the IDT.
• Provides supervision to Hospice Aides/Homemakers and educates families.
