Job summary

Swan Hospice is actively seeking Registered Nurses (RN) to join our team in New Jersey. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes.

As an RN, you will oversee primary caseloads, perform initial and ongoing nursing assessments, collaborate with the Interdisciplinary Team, and support families and caregivers while supervising aides to ensure compassionate, standards-based care.

Qualifications

• Must be a Registered Nurse licensed in New Jersey.
• At least 1 year of general nursing experience.
• Experience in hospice, home health, medical, surgical or critical care preferred.
• Understanding of hospice philosophy and team-based care.
• Ability to work within an interdisciplinary team.
• Valid driver’s license and reliable transportation.

Responsibilities

• Oversees primary caseload of hospice patients and provides skilled nursing care.
• Performs initial and comprehensive nursing assessments and plans of care.
• Reassesses nursing needs and revises care plans as patient needs change.
• Teaches, counsels, and supports patients, families, and staff on nursing care.
• Collaborates with the IDT to ensure comprehensive patient and family care.

Education

Bachelor’s degree in Nursing
