Job summary

Three Oaks Hospice in Illinois is seeking a registered nurse to provide skilled nursing care to patients at home and coordinate with the interdisciplinary team to ensure compassionate end-of-life care.

Responsibilities include assessments, care planning, patient/family education, and on-call coverage as part of a dedicated hospice program that supports patients and families through difficult times.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver's license.
• Two years RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; MS Office preferred.
• Strong interpersonal and communication skills with patients and families.
• High standard of ethics and compliance; ability to cope with family stress.

Responsibilities

• Assess patients and document health status, risks, and needs.
• Coordinate care with the interdisciplinary team and families.
• Educate patients and families about end-of-life care and planning.
• Initiate and update individualized care plans and goals.
• Provide on-call nursing services as assigned.
• Use prescribed procedures to deliver skilled nursing care.
• Maintain accurate progress notes and communicate changes.
