Job summary

Three Oaks Hospice is seeking a Registered Nurse to provide skilled nursing care in patients' homes or residences across Texas. The role coordinates care with the interdisciplinary team and supports patients and families through advanced disease and end-of-life needs.

You will assess, plan, implement, and document care; perform on-call duties as assigned; maintain CPR certification; and uphold high ethics and compliance while delivering compassionate, patient-centered care.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal and communication skills; ethics and compliance.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Educate patients and families about the disease process, end-of-life care, nutrition, and self-care techniques.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Prepare clinical notes demonstrating progress toward goals.
• Coordinate all patient/family services with the interdisciplinary team.
• Inform physicians and staff of changes in patient needs and outcomes.
• Determine scope and frequency of services based on acuity and needs.
• Assess caregiver ability to meet patient needs and provide ongoing support.
