Job summary

Three Oaks Hospice, Inc. is seeking a Registered Nurse to administer skilled nursing care for patients in their homes and coordinate with the interdisciplinary team. The role emphasizes patient comfort, education, and caregiver support.

The RN will perform comprehensive assessments, develop and revise the plan of care, and coordinate services with physicians, families, and referral agencies to ensure quality end-of-life care in a home setting.

Qualifications

• Graduate-level nursing degree not required? (use statements from HTML)
• RN license and current driver's license required
• Two years’ RN experience in clinical care; hospice/home health preferred
• Current CPR certification required

Responsibilities

• Perform initial and ongoing nursing assessments for patients in their home.
• Coordinate care with the interdisciplinary team and referral sources.
• Educate patients and families on disease process, care plans, and self-care.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Document progress notes showing goal achievement.
• Coordinate services with IDT and refer to additional resources as needed.
