Job summary

Three Oaks Hospice is seeking a compassionate Registered Nurse to provide skilled nursing care for patients in their homes, coordinating with an interdisciplinary team and families. The role emphasizes patient-centered assessment, education on end-of-life care, and case management to ensure comfort and quality of life.

The ideal candidate holds an RN license, CPR certification, and 2 years of clinical RN experience, with hospice or home health preferred.

Qualifications

• Graduate of an accredited program in nursing.
• Current state RN license and driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Strong computer skills; MS Office experience preferred.
• Excellent interpersonal, communication, and organizational abilities.

Responsibilities

• Administer skilled nursing care for patients in their home or residence.
• Coordinate care with Interdisciplinary Team, patient, family, and referral sources.
• Educate patient and family about disease process, end-of-life care, and care plans.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Maintain progress notes and document outcomes toward goals.
• Coordinate services with IDT and referrals; adjust plan based on acuity.
• Identify changes in patient needs and communicate with physicians and staff.
• Assess caregiver ability to meet patient needs and arrange support as needed.
• Provide on-call coverage to patients and families as assigned.
