Job summary

Three Oaks Hospice, Inc. seeks a registered nurse to provide skilled nursing care in patients' homes, coordinating with an interdisciplinary team and family to plan and deliver end-of-life care.

The role involves comprehensive assessments, ongoing care coordination, and education for patients and caregivers, with on-call responsibilities as assigned. Prior hospice or home health experience is preferred; CPR certification is required.

Qualifications

• RN with nursing degree and license, eligible to practice in the state.
• Two years of RN experience in clinical care; hospice/home health preferred.
• Proficient in computer use and basic Microsoft Office.

Responsibilities

• Perform initial and ongoing nursing assessments in patients' homes and coordinate care with the IDT.
• Develop and revise the plan of care in collaboration with patient/family and team.
• Educate patient/family on disease process, self-care, and end-of-life care; provide staff training as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Document progress notes showing progress toward goals.
