Job summary

Three Oaks Hospice, Inc. seeks an experienced registered nurse to provide skilled nursing care to patients in their homes, coordinating with the interdisciplinary team and families to ensure timely, compassionate care.

You will perform comprehensive assessments, implement the plan of care, educate patients and families on end-of-life care, and document progress. On-call coverage is required.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computer and Microsoft Office.
• Strong interpersonal, communication, and organizational skills.
• High standard of ethics and compliance.

Responsibilities

• Administer skilled nursing care for clients in their home.
• Coordinate care with the interdisciplinary team and families.
• Perform comprehensive assessments and update the plan of care.
• Provide education to patients and families on disease, self-care, end-of-life care.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes and document care.
• Manage on-call responsibilities for patients and families.
• Communicate changes in patient needs to the physician.
• Ensure appropriate admission and re-certification criteria.
• Other duties as assigned.
