Job summary

Three Oaks Hospice is seeking a Registered Nurse to administer skilled nursing care in patients' homes, coordinate with the interdisciplinary team, and oversee care plans. The role requires an Associate or Bachelor's degree in Nursing, current RN license, CPR, and at least two years of clinical RN experience; hospice/home health preferred.

Strong communication and organizational skills are essential. On-call duties and coordination across IDT require empathy and resilience.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients’ physical, functional, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and revise the plan as necessary.
• Educating the patient/family regarding disease process, self-care techniques, end-of-life care; provide training to staff as needed.
• Initiating appropriate preventive and rehabilitative nursing procedures.
• Preparing clinical and progress notes showing progress toward established goals.
• Coordinating all patient/family services and prioritization of needs with the IDT.
