Qualifications

• RN with state license and CPR certification.
• 2 years’ nursing experience in clinical care; hospice/home health preferred.
• Proficient computer skills and ability to use standard office software.

Responsibilities

• Administer skilled nursing care in patients' homes.
• Coordinate care with the interdisciplinary team and family.
• Manage care plans, assessments, and documentation.
• Provide on-call coverage as assigned.
• Educate patients and families about disease, end-of-life care, and self-care.
