Job summary

Three Oaks Hospice is seeking an experienced RN to administer skilled nursing care in patients’ homes, coordinating with the interdisciplinary team and families. The role emphasizes end-of-life care, patient comfort, and care planning.

You will assess needs, educate families, and lead care coordination while maintaining clinical documentation and on-call responsibilities. RN license, CPR, and strong communication are essential for success.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver's license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office preferred.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs and ADLs.
• Coordinate care with the Interdisciplinary Team and family.
• Educate patient/family on disease, end-of-life care, and self-care.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical notes showing progress toward goals.
• Coordinate services and manage prioritization with IDT.
• Use case management and refer to additional services as needed.
• Inform physician of changes in patient needs and outcomes.
• Determine scope/frequency of services based on acuity.
• Assess caregiver capacity for patient needs.
• Provide on-call coverage as assigned.
• Other duties as assigned.
